We are glad that you are considering Hill Top Home of Comfort as your new home. To assist you in planning for you future or your loved one's stay here, following is a checklist of things that need to be considered and/or taken care of before you, or your loved one moves in:
REQUIREMENTS TO BE MET PRIOR TO ADMISSION:
1. NURSING HOME APPLICATION. We prefer that this be completed before you move into Hill Top Home.
2. NURSING HOME HISTORY AND PHYSICAL. It is a must that you have a local physician who can come to Hill Top Home to see you while you live here. To assist you in finding a doctor, refer to the list of doctors who make visits to Hill Top below. After you have decided upon a doctor, you need to make an appointment with him/her WITHIN 5 DAYS OF YOUR ADMISSION to Hill Top. At this time, he/she will complete a written history and physical.
It is very important that your doctor completes the history and physical 5 days before you come in. Without it, we cannot admit you to Hill Top Home.
BRING IN THE FOLLOWING PREFERABLY BEFORE OR ON THE DAY OF ADMISSION:
1. Social Security Card.
2. Medicare Card.
3. Medicare Part D (drug plan) card.
4. Copy of Living Will, Durable Power of Attorney or Power of Attorney, if you have any of these Advance Directives in place. (We can make copies for you)
5. Long Term Policy and information. (We will make a copy for our files).
6. Recommended admission items, for example, clothing, etc.- attached list.

All clothing must be Permanent Press, machine washable and machine dryable. If the resident wishes to retain items labeled "dry clean," the family or resident is responsible for taking items to the dry cleaner and resulting charges. We can not be responsible for dry cleanable clothing that is accidentally laundered.

If you plan to bring in chairs for your loved ones, there are some things that we would like you to consider before you do this. Recliners and other chairs can pose a fall risk to residents if they are not stable or have openings in which a resident's feet may become trapped. Swivel chairs can be extremely hazardous if a resident would attempt to use them as support when trying to stand. There are no swivel chairs in the public area of the Home.
Recliners often have a rocking feature. This can increase the resident's risk of falls if they try to use the recliner as support as they stand. We also know rocking can be soothing for some residents. Recliners are often used to elevate the resident's legs and feet if the resident has difficulty with swelling of the feet. As a resident or family member you must weigh the risks and benefits. If you wish to use a recliner, we do recommend recliners which are closed between the seat and foot rest to prevent entrapment of the foot or leg as a resident stands.
The size of the room and storage areas must be kept in mind when belongings are brought to the Home. State law requires that clothing hang freely and that clutter is kept at a minimum in order that proper cleaning and disinfecting procedures can be followed.
Garments and other items brought to the Home are marked with the resident's name and entered on their inventory list of personal belongings. Garments brought in are given to the Charge Nurse or Director of Environmental Services with the resident's name on the wrapping containing the items. The resident's caretaker is requested from time to time, to remove clothing and other items that are no longer in use in order to keep closets, drawers and the room neat and free of clutter. Be certain to document items taken or discarded, along with the resident's name so their inventory list is kept up-to-date. Please leave the list with the Charge Nurse or Director of Environmental Services.
